If you miss the enrollment period, you must wait until the next open enrollment to make changes, unless you experience a qualifying event and report it within 30 days.

What’s Changing

As part of SCAN’s ongoing strategy to continuously provide a robust benefits program, 2025 changes have been made to our 2025 coverage options. SCAN has made the decision to keep employee contributions for medical, dental, and vision the same for 2025 and will be absorbing cost of increased premiums.

Program What’s Changing in 2025
Attestation

Employees must attest they are not covered under any other medical, dental, and/or vision plan to enroll in SCAN’s plan; violations may lead to disciplinary action, including termination.

Medical

There are slight changes to medical plan designs to ensure quality of coverage while maintaining sustainable costs for the organization. Deductibles, copayments, and annual out-of-pocket maximums will increase.

There will be expanded access to artificial insemination for all plans.

Behavioral Health

Aetna EAP and Headspace will be replaced by Modern Health, a mental wellness platform that provides personalized resources, services, and tools to support your emotional well-being — benefits include:

  • Six one-on-one sessions with certified mental health, professional, or financial well-being coaches
  • Six one-on-one sessions with licensed clinical therapists
  • Unlimited group support sessions (called Circles)
  • A library of guided meditations & self-paced digital courses
  • Ongoing well-being assessments to check-in on your well-being over time.
Life and AD&D Insurance

Prudential will replace Hartford as our life and AD&D insurance provider. There will be no changes to your coverage.

  • During this Open Enrollment, employees will have a one-time opportunity to purchase or increase their life insurance coverage for themselves and their spouses up to the guaranteed issue limit with no evidence of insurability required.
  • Voluntary life insurance premiums will decrease for most tiers.
Disability Insurance

Prudential will replace Hartford as our disability insurance provider. There will be no changes to your coverage.

Health Savings Account (HSA)

HSA contribution limits will increase to $4,300 for employee-only coverage and $8,550 if you cover dependents.

Flexible Spending Accounts

The Health Care and Limited Purpose FSA contribution limit is $3,300.

There are no changes to the Dependent Care FSA contribution limit.

Dental

There will be no changes to premiums in 2025.

The following provisions have been updated:

  • Late Entrants will now be eligible for basic/major services, including orthodontia
  • Coverage for missing teeth prior to the effective date of coverage
  • Coverage for orthodontia treatment that began prior to the effective date of coverage
  • Waiting period for eligible dental services no longer apply
Vision

There will be no changes to coverage or premiums in 2025.

Supplemental Medical

There will be no changes to your Accident Insurance, Critical Illness Insurance, or Hospital Indemnity Insurance coverage in 2025.

Voluntary Benefits

There will be no changes to Pet Insurance or the Legal Plan in 2025.

Savings & Spending Accounts

Health Care and Limited Purpose FSA – The contribution limit is $3,300.

Dependent Care FSA – The contribution limit is $5,000. You must re-enroll to participate in 2025.

Health Savings Account – Your 2024 election will roll into 2025. The contribution limit in 2025 is $4,300 for employee only coverage and $8,550 if you cover dependents. You can change your 2025 contribution during open enrollment or at any time during the year without a qualifying life event.

Life Insurance

Prudential will replace Hartford as our basic and voluntary life insurance provider. There will be no changes to your coverage.

As part of this transition from Hartford to Prudential, Prudential is offering two special enrollment opportunities for voluntary life insurance:

  • During this Open Enrollment, current employees can purchase or increase life insurance coverage for themselves and their spouses up to the guaranteed issue limit with no evidence of insurability required.
  • Employees should also consider enrolling in the new voluntary AD&D plans. Coverage must be equal to voluntary life elections.

Disability Insurance

Prudential will replace Hartford as our Disability Insurance provider in 2025. There will be no changes to your coverage. Eligible employees will continue to be automatically enrolled in short-term and long-term disability insurance at no cost to you.

Wellness

Modern Health will be replacing Aetna EAP and Headspace. This new program offers personalized care designed to meet your unique needs, providing easy access to a wide range of support. Through SCAN's program, you'll receive 12 free sessions (six therapy + six coaching) for both you and your dependents—completely at no cost to you.

Enrollment Details

Who Can Enroll in SCAN Benefits?

All active full-time employees who are scheduled to work 30 or more hours per week are eligible to participate in SCAN’s benefits program. You may also cover your eligible dependents under SCAN’s medical, dental, vision, and life benefits. A limited selection of benefits are provided for SCAN Temps scheduled to work 30 or more hours per week.

Your Eligible Dependents Include:

  • Spouse or domestic partner (state registration is required)
  • Your child(ren) and your covered spouse’s or domestic partner’s child(ren) up to age 26
  • Disabled children who became disabled on or before age 26 

Documentation is not required at the time of enrollment. However, SCAN will perform audits periodically to verify dependent eligibility.

Follow these step-by-step instructions to make your elections and make changes to your 2025 elections.

Pro Tip! Remember to review your confirmation statement for accuracy and save it for recordkeeping purposes.

Important: After Open Enrollment ends, you cannot change your benefit elections unless you experience a qualifying life event, such as a birth, marriage, loss or gain of coverage, and report your event to HR within 30 days of the event.

  • Re-enroll in Health Care, Limited Purpose, and Dependent Care FSA — they don’t carry over to the next year!
  • Consider enrolling in voluntary life for you and your spouse or increasing your coverage up to the guaranteed issue amount.
  • Consider newly enrolling in voluntary AD&D for you and your family.
  • Update your beneficiaries in Employee Central and Empower
  • Finalize your elections no later than Friday, November 15.

If you are currently enrolled in a medical HMO or dental DMO with SCAN and wish to update your primary care provider, you must contact Aetna directly to make changes.
