You have 30 days beginning on your date of hire to elect your health and welfare benefits. Coverage for most plans go into effect on the first of the month following your date of hire.

Enroll today! You have 30 days beginning on your date of hire to elect your SCAN health and welfare benefits. If you do not take action, you will be enrolled in the SCAN-paid plans only.

Your next opportunity to enroll in benefits will be during SCAN’s annual Open Enrollment held each year in the fall, unless you experience a qualifying life event such as a birth or a marriage. If you experience a qualified life event you have 30 days beginning on the date the event to make changes to your benefits.

Enrollment Attestation

An employee enrolled in any other medical, dental or vision plan, including, but not limited to a spouse's or partner's plan, should decline enrollment for medical, dental and/or vision, as appropriate. SCAN will no longer cover employees who have elected and are also enrolled in coverage outside of SCAN, and may take appropriate action, which may include: (i) request that the employee reimburse SCAN for all costs associated with employee's coverage; and/or (ii) corrective action, up to and including termination. Employees who experience a qualifying event during the year, such as loss of outside medical coverage, may reach out to HR to find out about coverage options.

By enrolling in SCAN's medical, dental, and/or vision plan(s), you confirm that you will not be enrolled in any other medical, dental or vision plan outside of SCAN, including, but not limited to, a spouse's or partner's plan. You understand the consequences of misrepresentation, including the potential for corrective action, up to and including termination, as well the requirement to reimburse SCAN for any costs incurred. You acknowledge your responsibility to inform HR of any qualifying events that may affect my coverage.

Dependent Eligibility

By enrolling your eligible dependents in coverage, you attest they are eligible based on plan guidelines. Regular audits may be performed. In the event you enroll an ineligible dependent, there may consequences of misrepresentation, including the potential for corrective action, up to and including termination, as well as the requirement to reimburse SCAN for any costs incurred.

Eligible Dependents include:

  1. Spouse – Your legal spouse.
  2. Children – Biological, adopted, or stepchildren, until they reach the age of 26.
  3. Domestic Partners – Must be registered by the state you reside in (documentation required).
  4. Disabled Dependents – Children over age 26 who are disabled (documentation required).

How to Enroll

After reviewing your benefit options, make your enrollment selections within 30 days of your date of hire. Your benefits are effective the first of the month following your date of hire.

Enrolling through Employee Central (UKG) is Easy!

Employee Central (UKG) will guide you through your enrollment every step of the way. For the best experience, we recommend using Chrome as your web browser.

Make Your Elections

To select your benefits from the Employee Central homepage:

  1. Click Menu > Myself > Benefits > Manage My Benefits. Then, you’ll be able to choose a path to begin selecting your benefits.
  2. Choose your elections.
  3. Click “Check Out” to complete your enrollment.
  4. Print a copy of your confirmation statement for your records.

Important Reminders!

  • Be sure to provide the necessary dependent information to enroll eligible family members.
  • Make sure you’ve provided beneficiary information for your life insurance.
  • Keep in mind, your benefits are effective the first of the month following your date of hire.

2. Designate Your 401(k) Deferrals

Save for the future through SCAN’s 401(k) program through Empower Retirement. You can designate your deferrals after you receive your first regularly schedule paycheck. If you don’t take any action by designating a deferral or opting out within 45 days of your hire date, your contribution will be automatically set at 4% of your eligible compensation.

Employees are eligible for SCAN’s employer match and safe harbor contribution beginning the first of the month following 60 days of employment.

How to Enroll

You can easily enroll in the SCAN 401(k) Plan and manage your account online or by phone:

  • Visit the Empower Retirement website – Enroll online at your convenience, any time of day or night. You may enroll once you have received your first regularly scheduled paycheck.
  • Call 1-866-467-7756 Representatives are available to help you Monday through Friday, from Weekdays 5am to 7pm Pacific; Saturdays 6am to 2:30pm Pacific.
