You have 30 days beginning on your date of hire to elect your health and welfare benefits. Coverage for most plans go into effect on the first of the month following your date of hire.

1. Enroll in Health & Welfare Benefits

Enroll today! You have 30 days beginning on your date of hire to elect your SCAN health and welfare benefits. If you do not take action, you will be enrolled in the SCAN-paid plans only.

Your next opportunity to enroll in benefits will be during SCAN’s annual Open Enrollment held each year in the fall, unless you experience a qualifying life event such as a birth or a marriage. If you experience a qualified life event you have 30 days beginning on the date the event to make changes to your benefits.

Who Can Enroll in SCAN Benefits?

All active employees who are scheduled to work 30 or more hours per week are eligible for SCAN’s benefits. You may also enroll your eligible dependents under medical, prescription, dental, vision, and life benefits. A limited selection of benefits are provided for SCAN Temps scheduled to work 30 or more hours per week.

Your Eligible Dependents Include:

  • Spouse or domestic partner (state registration is required)
  • Your child(ren) and your covered spouse’s or domestic partner’s child(ren) up to age 26
  • Disabled children who became disabled on or before age 26

What Documents Do I Need to Verify My Family Members?

You must upload documentation in Employee Central within 14 days of completing your enrollment to verify your dependents’ eligibility. The types of acceptable documentation are indicated below:

  • Spouse - marriage certificate
  • Domestic Partner – proof of state-registered domestic partnership
  • Children - birth certificate, decree or order of adoption.

A copy of your current tax return may be acceptable if it lists your name and the family member as a dependent.

If documentation is not provided, your dependent’s coverage will not go into effect.

How to Enroll

After reviewing your benefit options, make your enrollment selections within 30 days of your date of hire. Your benefits are effective the first of the month following your date of hire.

Enrolling through Employee Central (UKG) is Easy!

Employee Central (UKG) will guide you through your enrollment every step of the way. For the best experience, we recommend using Chrome as your web browser.

Make Your Elections

To select your benefits from the Employee Central homepage:

  1. Click Menu > Myself > Benefits > Manage My Benefits. Then, you’ll be able to choose a path to begin selecting your benefits.
  2. Choose your elections.
  3. Click “Check Out” to complete your enrollment.
  4. Print a copy of your confirmation statement for your records.
  5. Verify your dependents by uploading the required documentation to verify their eligibility within 14 days of completing your enrollment.

Important Reminders!

  • Be sure to provide the necessary dependent information to enroll eligible family members.
  • Make sure you’ve provided beneficiary information for your life insurance.
  • Keep in mind, your benefits are effective the first of the month following your date of hire.

2. Designate Your 401(k) Deferrals

Save for the future through SCAN’s 401(k) program through Empower Retirement. You can designate your deferrals after you receive your first regularly schedule paycheck. If you don’t take any action by designating a deferral or opting out within 45 days of your hire date, your contribution will be automatically set at 4% of your eligible compensation.

Employees are eligible for SCAN’s employer contributions beginning the first pay period following 90 days of employment. There are two types of contributions:

Safe Harbor Contribution 5%
Employer Match 50% of the first 4%

How to Enroll

You can easily enroll in the SCAN 401(k) Plan and manage your account online or by phone:

  • Visit the Empower Retirement website – Enroll online at your convenience, any time of day or night. You may enroll once you have received your first regularly scheduled paycheck.
  • Call 1-866-467-7756 Representatives are available to help you Monday through Friday, from Weekdays 5am to 7pm Pacific; Saturdays 6am to 2:30pm Pacific.
